2010 Dress Up 2 Cure Campaign Incentive Program Eligibility Rules
1. There will be THREE total winners. All winners will be selected at random from a pool of entries for each individual prize.  One entry is given
per team per $100 raised in the campaign. Entries may be placed into one raffle for one item, or dispersed among the three raffles as desired.
Prize 1 will be an Air Tran custom vacation package valued at $1,500.
Prize 2 is an UP ‘N Go Pediatric Walker valued at $3,000.
Prize 3 is an $8,500 gift certificate to SleepSafeBeds.com to design a custom bed.
2. Any person who creates a webpage on www.FirstGiving.com/dressup2cure and raises at least $100 during the Dress Up 2 Cure Campaign
through May 15, 2010 will be eligible.
3. To verify participation in the raffle, a campaign participation form must be completed, including a person’s name, address, telephone
number.  The cutoff date for donations is midnight EST on May15, 2010.  Participants have through June 1, 2010 to communicate where they
would like their entries placed.  Winners for each of the three prizes will be selected at random at noon EST on June 15, 2010.
4. Members of the staff, Board of Directors of Girl Power 2 Cure, Inc., and their immediate family members are not eligible. Immediate family
members include spouses, parents, and children.
5. Winners are required to provide proper identification and verification upon presentation of prizes.
6. By entering their name, each winner agrees that Girl Power 2 Cure, Inc.  may use their name and/or photograph for publicity purposes upon
presentation of prizes.
7. No cash equivalent will be provided in lieu of the prizes.
8. Winners will be required to pay any applicable taxes relating to transactions including but not limited to federal and state income taxes.
9. Participants must be 18 years of age or older.
10. Participation in this raffle may affect the tax deductibility of your individual donation if you make one to your team; therefore, you should
consult your tax advisor for more information.
HOW THIS WORKS:

  1. Sign up for a NEW TEAM fund raising page through our
    FirstGiving portal.
  2. Fill out the form on this page to enter into our "Incentive
    Prize Program."
  3. On May 15, 2010, we will gather final tallies for your team
    which includes all fund raisers raising money for your
    team.
  4. For every $100 your team has raised, you will get one
    entry in the prize raffle.
  5. There are THREE PRIZES, so you can put all of your
    chances towards one prize, or spread them out. We will
    take your request via email to prizes@dressup2cure.org.
  6. Winners will be announced by video at 12 pm EST on
    June 10, 2010.
